- Be able to give a urine sample.

- If previously given an order for blood work, make sure that you get them done at least a few days prior to your appointment. That will help us better assess your needs.

Patient Billing & Payment Process
Please bring your insurance cards with you to all appointments. We participate in most insurance plans. Since each plan is different, we ask that you check with your plan administrator about your insurance guidelines before your appointment. Co-pays are expected at the time of your appointment. We will file your insurance claim for you.
If we do not participate with your insurance carrier, or if you do not have insurance, payment is expected at the time of your appointment.
